How do I find out who my GP is???

I'thousand filling in a medical form for a course, and it wants to know, among other things, the details of my GP.

How practice I find this out?

I oasis't been to see a doc for about eight years. I couldn't fifty-fifty name a doctor working at my local surgery.

  • I just normally write downwardly the surgery name - I never commonly write the doctors proper noun cause 1. I don't know who he is and 2. I've never ever been to see my actual doc.

    You could call the surgery though, the receptionist should exist able to tell you

  • Are you lot registered with a clinic/GP?

    If not and then find your nearest one and sign up. When y'all sign upwardly they will see if yous are signed up elsewhere.

    If y'all are and then only telephone call them for the details of your GP.

  • Telephone call the surgery where your records are held. You will have a physician assigned to you. They can tell you.

  • I just put the surgery name normally, I never see the same md anyway.

  • I only ever put down the name and address of the medical do - I rarely go to the GP and never see the same person twice. If for any reason they wanted a medical study, they would accept to get your signed consent beforehand anyway.

    No one actually has a personal gp anymore! just put the practise name :)
